COMMON REQUIREMENTS

1. Passport (Signature of the holder required)

2. Application Form (A recent facial photo, 4.5 x 3.5 cm, must be attached)

3. Request for Multiple-Entry Visa

   • If the option for [sufficient financial capacity] is selected, the embassy will review and determine whether the applicant qualifies for a Multiple-Entry Visa for Temporary Visitors or a Multiple-Entry Visa for Philippine Nationals with Considerable Financial Capacity.

4. PSA-issued Birth Certificate and Marriage Certificate (for married applicants), issued within 1 year.
   Not required if there is a used Japan visa in the passport.

ADDITIONAL REQUIREMENTS

• If the PSA-issued certificates (Birth/Marriage) are unreadable, please submit the certificates issued by the Local Civil Registrar.

• If the Birth Certificate is a Late Registration, please submit the Baptismal Certificate and School Record (Form 137).

• If there is no record of the Birth/Marriage in the PSA, submit the Birth Certificate issued by the Local Civil Registrar along with a Negative Certificate issued by the PSA.

CONDITIONS AND SPECIFIC REQUIREMENTS

I. Applicants with High Income

1. Bank Certificate (Balance from the last six months must be shown)

   • If the Average Daily Balance (ADB) for the past six months is not indicated, a bank statement must be submitted to prove transactions during that period.

2. Tax Payment Certificate issued by the Bureau of Internal Revenue (BIR) (Copy acceptable)

   • Form 2316 must be signed by both the Employer and Employee.

   • For business owners, proof of actual tax payment is required in addition to the BIR Tax Payment Certificate (Copy acceptable).

3. Employment Certificate (Must include period of employment, salary, and position)

   • For business owners, submit the Department of Trade and Industry “Certificate of Business Name Registration” and the Mayor’s Permit from the City Hall.

   • If the applicant is unable to submit the documents in (2) and (3) for reasons other than being a retiree, full-time housewife, or unemployed, a letter of explanation must be submitted.

   • For students, submit a copy of the School ID or Certificate of Enrollment.

II. Spouse or Child of (I)
Proof of relationship with the applicant in (I) above:

• PSA-issued Birth Certificate and Marriage Certificate (if applicable).

• Passport and multiple-entry visa copy or other relevant documents of the applicant in (I) above.
