Tourist Visa Requirements for Filipino Employees:

1. Application Form {for Visayas handwritten or type written and for Luzon & Mindanao must be type written and printed in A4 size)

2. lpiece of Passport size colored picture with white background

3. Original Passport with signature (should be valid for more than 6months)

4. Photocopy of Passport Bio-page (page 2)

5. Photocopy of used visa/sand arrival stamps to OECO member countries for the last 5 years; Austria, Australia, Belgium, Canada, Chile, Estonia, France, Germany, Greece, Hungary, Denmark, Iceland, Israel, Italy, Japan, South Korea, Spain, Sweden, Turkey, United Kingdom, United States, Slovenia, Portugal, New Zealand, Czech Republic, Finland, Switzerland, Me>rico, Luxembourg, Norway, Scotland Republic & Slovak ( If Applicable)

6. Original Certificate of Employment

{including applicant's position, date hired, compensation, address, office landline number, e-mail address)

7. Original Personal Bank Certificate

(must include account type, current balance, account opening date, ADB)

8. Bank Statement

{original or certified true copy of bank statements/passbook for the last 3 months)

9. Photocopy of ITR {Income Tax Return) or Form 2316 (Checked by BIR with stamp) latest

10. A letter of Attorney for Visa Application (we will beproviding the form andnot necessary to be notarized)

11. Original PSA Birth Certificate(If travelling with Family) must be issued within 3months

12. Original PSA Marriage Certificate{lf travelling with Spouse) must be issued within 3months

13. Copy or PRC Card or IBP Card (If applicable only)

5. 6. Photocopy of Business Registration from SEC or DTI

7. Photocopy of Business Permit or Mayor's Permit

8. Original Personal Bank Certificate

(must include account type, current balance, account opening date, ADB)

9. Bank Statement

(original or certified true copy of bank statements/passbook for the last 3 months)

10. Photocopy of ITR (Income Tax Return) or Form 1700, 1701, 1702 (Checked by BIR with stamp) latest

11. A letter of Attorney for Visa Application (we will be providing the form and not necessary to be notarized)

12. Original PSA Birth Certificate(lf travelling with Family) must be issued within 3months

13. Original PSA Marriage Certificate(lf travelling with Spouse) must be issued within 3months
